How do I add, change or remove a beneficiary?

First let us know you’d like to add or update the beneficiary on your checking, savings or certificate of deposit account. Next, we'll need your written request to complete it. Start by visiting your local U.S. Bank branch or calling 800-USBANKS (872-2657). We accept relay calls. 

How to add or change a beneficiary

  1. Speak to a banker and let them know the following:
    • What you’d like to do with the beneficiary on the account.
    • If the beneficiary is on, or should be on, multiple accounts. We'll ask for the last four digits of each account number impacted.  
  2. Provide the beneficiary’s name, date of birth, address, phone number and SSN*. 
  3. A form will be created with this information and mailed to you.
  4. After you get the form, review it for accuracy. If correct, sign the form.
  5. Return it to the following address or fax number:

How to remove a beneficiary

  1. Speak to a banker and let them know the following:
    • What you’d like to do with the beneficiary on the account.
    • If the beneficiary is on multiple accounts. We’ll ask for the last four digits of each account number and review for potential impact.
  2. Create and sign a letter asking to remove the beneficiary, including their name, all account numbers impacted, and the effective date of removal.
  3. Send it to the following address or fax number:

  • It takes 10 business days to process the maintenance, starting when we receive the written request to add, change, or remove a beneficiary.
  • Beneficiaries don't have signing authority. This means, they don't have to sign a form to add, change, or remove their information. They're also not required to be present at the time of designation or removal.

*If it's a business or trust (including charities or churches), we'll need the business name, address, phone number, and TIN.
