U.S. Bank Business Essentials® acknowledgments

You reviewed and agreed to the following in your Business Essentials application:

• I’m authorized to submit this Application and all information provided in this Application is true, accurate and complete.
• I authorize U.S. Bank and Elavon, Inc. to obtain consumer reports on me in connection with evaluating this Application. Once this Application is approved, I authorize U.S. Bank and Elavon, Inc. to obtain consumer reports on me on a monthly basis or at any time for purposes of credit monitoring, account review or any other legitimate purpose related to the Business Essentials payment processing services. U.S. Bank and Elavon, Inc. are also authorized to make inquiries to review and validate employment history or any other information contained within this application on a monthly basis.
• If I haven’t ordered a card reader through this Application, I understand that I have 90 days following the approval of this Application to enable my payment processing services. If I don’t, I’ll need to re-apply for payment processing services.
• If I’ve ordered a card reader through this Application, I must complete a card processing transaction within 180 days following the approval of my account. If I don’t, I’ll lose access to processing functionality and I’ll need to re-apply for payment processing services.
• My company, regardless of transaction volume, must comply with the requirements of the Payment Card Industry Data Security Standards (PCI DSS). My company may be eligible for Data Breach Financial Assistance Coverage following account approval. (See the PCI Compliance Program Overview for details and conditions, found in the U.S. Bank Business Essentials Payment Processing Terms of Service and Operating Agreement (PDF).)
• In addition to any termination rights in the documents provided here, if any one of the U.S. Bank Business Essentials services is terminated (i.e., deposit account, payment processing, talech), then U.S. Bank, Elavon, Inc., or Talech, Inc., as applicable, may terminate the remainder of the U.S. Bank Business Essentials services.
• My company and its representatives authorize U.S. Bank, Elavon, Inc., and talech, prior to their acceptance of our Application and from time to time thereafter, to investigate: (a) the company’s individual and business history and background, (b) each such representative and any other officers, partners, proprietors, and/or owners of the company, and (c) other background investigation reports on each of them that U.S. Bank considers necessary to review the acceptance and continuation of this Application.
